PENTA accepts proposals throughout the year and requires a minimum of four weeks for review and decision. Please make sure to complete the request form below, and fax (508.616.0770) or email the following documents:

501 (c)(3) Letter of Determination from the IRS
Most recent Annual Report
Names and Affiliations of the organization's board of directors
Supportive literature pertaining to your request.
