Guidelines on How to Apply
PENTA accepts proposals throughout the year and requires a minimum of four weeks for review and decision. Please make sure to complete the request form below, and fax (508.616.0770) or email the following documents:

• 501 (c)(3) Letter of Determination from the IRS
• Most recent Annual Report
• Names and Affiliations of the organization's board of directors
• Supportive literature pertaining to your request.
